I recommend watching the one with bullet adjust if you need a video walkthrough; the "no bullet adjust" version is more for show-off and entertainment purposes, and is not really a walkthrough.

Yes, that Scratcher is dying. Just leave before the death animation finishes, and Scratchers will respawn. You can even see me do this (though unintentionally) in my 10 star run.
I've done a lot of testing, and I've come to the conclusion that the best way to get out of enemy grabs is to mash r1+l1+x+square while rotating the analog stick. I'm fairly confident that the other buttons (triangle, circle, r2, l2) don't even assist at all in breaking free. It would surprise me greatly if there is a faster, better way to mash in this game. Trust me ;)
