23Sam is back! De Verenigde Staten is militair aanwezig in twee derde van de wereld. Een aantal staten heeft er genoeg van en brengt een terroristisch ultimatum genaamd 'Blacklist' op gang: een dodelijke aftelling naar een reeks terroristsiche aanvallen tegen Amerikaanse belangen. Sam Fisher is de leider van de nieuwe 4th Echolon eenheid: een geheime eenheid die enkel verantwoording aflegt aan de Amerikaanse president. Sam en zijn team moeten deze terroristen opsporen en het aftellen naar Blacklist stoppen voor het te laat is.
