This isn't always the 3rd long rest, but it was for me. There will be a time when it's too late to get this scene, but I'm unsure when. It's best to try for it right away after reaching the Grove. There are 3 ways to get it each with a dialogue variation, and 2 variations of scenery.

To get the version where he mentions the Gith Crèche, you must have Lae'zel with you and then talk to Zorru in the Grove (he's straight ahead past the part where you can recruit Wyll on the way to Sazza the goblin. In front of the building where Pandirna the paralyzed tiefling Pandirna is at). After that, talk to Lae'zel and then long rest.

To get the version where he mentions the druid, you must talk to Nettie in the Grove. She is inside the druid building (the building is near the sacred idol the druids are chanting around). Talk to her, mention the tadpole, and then you must take the Wyvern poison from her by swearing to take it if you feel you're starting to turn. This will be a -1 disapproval from Astarion if you accept, so you may want to leave him at camp for this interaction. After that, talk to her about finding Halsin and then long rest.

To get the version where he mentions the goblin priestess, you must talk Arka the tiefling out of killing the goblin Sazza. Then, talk to Sazza and she'll mention the priestess. Agree to get her out of there, unlock the cage, then take her safely outside the grove. You can either make her invisible or take her to the secret area with a cave that you have to jump to behind the cage she was in. Be careful, because the cave has 3 traps that shoot fire at you and they will kill her in 1 hit. You can disarm the 1st trap by clicking the blue glowing stone near the entrance once you get in. There will be a small fight as well. The 2nd stone switch is straight ahead after the fight, but you need to climb to reach hit. To get to the 3rd stone switch, head to the right after the fight and then jump down to an area close to the water. Be careful, since it's really easy to get shot here by the fire. Once that's done, get her safely to the exit of the cave, talk to her, and then long rest.

To get the special dialogue where you can say for him to not bite you again (04:49), just make sure that you get the bite scene first. You can get the bite scene as early as the 3rd long rest, or technically the 2nd if you skipped the 1st official one, "Roughing It," by going straight to the Grove (Maybe you can even get it as a 1st long rest, but I never tried).

To get the stature dialogue, you just have to be any short race. I used a mod that gave me a short race tag, so that's why my character looked the same.

To get the difference in scenery, just long rest outside for stargazing and inside for staring at the ceiling (long rest in the crypt, Underdark, or anywhere that has a camp indoors).
