Karai is the 3rd and final boss in the game. She is an unplayable character.
As you can see in the video there's a slight graphics flaw on the character select screen when she is highlighted, but in-fight she is perfectly controllable with no problems. She plays like any other normal character and even has her own special moves. Because she is a 'complete' and unique character, I'm surprised Konami didn't make her a hidden playable character.
===== About Karai =====
Karai is a character who has appeared in the TMNT comics and several other media as a high-ranking member of Shredder's Foot Clan. In the Genesis/Mega Drive version of TTF, her design is very different to the SNES version of Karai, as she's more 'feminine' in this one and doesn't wear trousers/pants. As the Genesis game aims for a 'less flamboyant' style, her moves aren't as crazy or dramatic as the SNES Karai, meaning that she's less cheap. However under CPU control, she is still hard to defeat.

---- When she is highlighted on the character select screen, there is a flawed graphic. This is obviously because she isn't meant to be selected in the game.
---- When you select her on the character screen, it seems to use the voice saying "seven".
---- Amazingly, if both players select her, she actually has a proper alternative colour palette. This suggests she may have been originally intended to be a playable character.
---- Her Scissor Kick move is performed via a charge motion, has a lowering forward velocity/trajectory and hits twice if done close to the opponent. Sounds similar to another well-known boss character's move...
