The following commands are prefixed with three letters which represent the settings of the command block. E.g IUN: = Impulse Unconditional Needs_Redstone. These prefixes need to be removed if copying the commands.

Commands used:

Setup:
IUN: scoreboard objectives add dropped.mojangBanner dropped:mojang_banner_pattern

Clean Up:
IUN: scoreboard objectives remove dropped.mojangBanner

Give Scroll:
IUN: give @p mojang_banner_pattern{display:{Name:'{"text":"Levitation Scroll","color":"gold","italic":false}',Lore:['{"text":"Drop for Levitation effect."}']},HideFlags:127,Enchantments:[{}],tagLevitation:true}
