Elden Ring's biggest balance issue in my opinion is the terrible downscaling on password summons but in general is the abundance of FP or rather the low cost of many unique skills, ashes of war, and spells. Skills are generally meant as a power move or mix up but because many are so cheap and easy to spam for a lot of damage a lot of players never even touch the R1 or R2 buttons until they run out of FP.
