Error #1 - Centre of Mass and parachute placement miss calculated causing return module to be incorrectly orientated upon parachute deployment.
Information:
A simple, natural step from putting things into orbit is to of course put people up there too. Now rockets have evolved to do this quite easily by this point however a brand new avenue of engineering is now involved, life support. In the game this is taken for granted as the only real requirement at this point is room for the crew to fit and to not crash and kill any would be space man.
This episode was made in the spirit of Vostok 1, the mission that Yuri Gagarin became the first space man in history.
For us the main thing to remember here is that Yuri Gagarin had balls! This wasn't some run if the mill situation. Yuri wouldn't even have any control over the flight it was a case of "You see that thing we designed to lunch nukes at people with? Well, you're gonna ride it."
Putting life into orbit is pretty incredible stuff but from now on that's the easy part. It would seem as though going to the Moon would be the next natural step but there will be a few other technicalities which would have to be ironed out first.
