In Chapter 11: Firefly Lab, Joel and Ellie finally arrive at the Salt Lake City hospital, where the Fireflies have established their base. Ellie is unconscious due to nearly drowning during a flooded section, and Joel is separated from her upon arrival.

Joel soon learns that the Fireflies intend to perform a surgery on Ellie to extract the Cordyceps fungus growing in her brain, which they believe can lead to a cure. However, the procedure will kill her. Devastated by this revelation, Joel refuses to accept it.

What follows is an intense and emotional sequence where Joel fights his way through the hospital, determined to save Ellie at all costs. He kills numerous Firefly soldiers, ultimately reaching the operating room and taking Ellie from the surgeons — even killing Marlene, the Firefly leader, in the process.

This chapter is the emotional and moral climax of the game, forcing players to confront the consequences of Joel’s love and protectiveness — and whether his actions are justified. It sets the stage for the game’s controversial ending and leaves a lasting impact on the player.
