In order to unlock the Oculus achievement, you must find the huge eye on the first floor, and allow it to kill you. There are a few ways to get to the first floor, so either go through room 21 on the second floor (the first way you access the first floor), use the discs to unlock the door to the first floor in the stairwell, or use the elevator. MAKE SURE TO SAVE BEFORE DOING THIS! You will have to die to unlock the achievement.

Once you have gotten to the first floor, keep wandering around until you see many doors on the top and bottom of a hallway. I don't know if the door that is unlocked is random, but just keep checking the doors until you are able to enter one, which leads you to the room with the giant eye. Once in the room, just stay inside long enough for the eye to kill you, and the achievement will unlock at the game over screen!
