Miko has four phases; her first phase (Good), her second phase (Evil), her third phase (with the turtle goons) and a final phase.

The Touhou Wiki has a full outline of the fight (including Miko's weaknesses, which actually change with each phase). All you really need to know is that you want to hit Phase 1 with direct/physical attacks, you want to hit phase 2 with magical attacks and you want nice and strong AOE's for phase 3. Miko's PAR resistance apparently drops in phase 4 too, so that's something to consider. Miko's weak elements are DRK, PHYS and MAG. DRK is highly recommended.

In my practice runs, my team struggled the most with phase 1. The difficulty was partly because of Miko's habit of using Refresh every four turns, but mostly because I didn't have a DRK element attacker who targeted DEF (a la Parsee, Kasen, Kogasa, Flandre etc.). Once I could breeze past phase 1, the rest of the fight became much easier. Yuyuko was a particularly good for phase 2.
