Paranautical Activity is more or less the binding of isaac in first person and it comes in a voxel like style. So you start in a labyrinth of predefined rooms - which change each start and defeat the enemies in these rooms, to unlock the doors. You also find special rooms with bosses or minibosses, that give you a bonus item (if you don't die of course ;)) and rooms where you can spend money for items. The defeated enemies drop money or health at random, so you should pick all up.
The game is pretty dark (lightlevel), which is kinda crappy if you play the game with the sun shining from the side on your monitor, but maybe other levels are a bit better lit.
