In order to get the ANTIVAXXER achievement, you must kill the second boss of the game, Big Bad Rona, WITHOUT using the vaccine. To use the vaccine, you must shoot the red targets as you are ascending the tunnel during the fight. Just avoid these at all costs, because shooting even one of them will null the achievement for that try. Go ahead and set the difficulty to Easy for this, as difficulty DOES NOT matter!

The best thing to do after gearing up and starting the fight is to use the Shotgun's secondary fire to grapple the rings a few times so you are above Rona, and have some good momentum going so you are ascending slowly after letting go of a ring. Quickly open the weapon radial, switch to the Gatling Gun, and place a tracer on Rona by using the Gatling Gun's secondary fire.

DO NOT UNDER ANY CIRUMSTANCES USE A DAMAGE MULTIPLIER ITEM! These are the crossed guns item, and when you use one, you will be unable to use your alt fire, which in turn prevents you from using your Shotgun's grappling hook! Avoid this item like the plague! Stick to armor and health items if you need to!

Once you have a tracer placed, just go to town with regular fire until the tracer runs out of time! While Rona has a tracer on it, all bullets fired will target where you stuck it, which not only kills it quickly, but also prevents you from accidentally hitting a red target. Rinse and repeat switching between Shotgun and Gatling Gun, and Rona will go down quickly, and you will have earned one of the rarer achievements in the game!
