Dark War certainly changes in tone going into E4, with a new set of enemies that dominates the entire episode, though we eventually also see earlier varieties as well. The real worrisome point, however, is that E4 clearly needs to outdo E3 in the boss department, and NME was already extremely unreasonable.
The initial boss fight in E4 is similarly tanky, to the point where players might question if it's ever going to end. There are not one, but two interesting twists at the end, however; the second arguably risks being overly clever, but I did find it very interesting.
Beyond E4, there are also a trio of very unorthodox warp-only maps. I legitimately have no idea how a couple of parts of Vomitorium are intended to work; as far as I could tell one area results in a softlock, and the final room is unsurvivable. I certainly remembered the final warp-only map from my childhood though.
