Presumably because he never changed into his werewolf form speaking to the village leader didn’t progress the quest. I had to get creative and attacking one of the villagers prompts them to change into werewolves and advances the story.

I was able to get into the caverns and escape the island. In fact I was able to do it twice since I wasn’t happy with how I did in the final werewolf encounter in Ulgoth’s Beard.
