In order to earn the What? There's Nothing Wrong With These... achievement, you must get a starter weapon, and finish the game with it still in your possession. In order to find one of the three starter weapons (sword, bow and shield), explore the area right below where you spawn in when starting a new run. There are 3 secret blocks that you can interact with that will reveal the 3 items. Grab any one of these items and continue on your run.

During your run, make sure to NOT replace the item you chose with anything else. You need to make sure to keep this through the run to the final boss and kill them while you still have it. Just make sure to have a good secondary weapon, and use the Scroll of Power upgrades found throughout the run to enhance the weapon. Having good items will also be very helpful.

Make your way to the final boss and kill them. The last attack that kills them does NOT have to be the starter weapon you chose, so don't feel like you have to even use it at all. Having it in your possession is all that matters. Once you kill the final boss, leave your body at the fountain and exit the final room. The achievement will unlock when the credits start to roll!
