In order to unlock the Hisashi OSHA achievement, you must die to every type of elemental explosive barrel. This achievement currently only has an unlock rate of 0.1% of Players. This is very easily done, and can all be done in the first level, maybe even the first room. It WILL take 3 runs, however, to complete, as you need to die to all elemental barrels.

There are 3 elemental explosive barrels in the game: Blue, which is electric. Red, which is explosive. Green, which is caustic/poison. In order for the green barrel to register, you MUST die to the explosion. Dying to the poison it pours out onto the ground (which deals DOT), will NOT count.

The easiest way to do this would be to just start a run, go to the first room, and kill ALL the enemies. Try to avoid allowing any of the elemental barrels to be destroyed during the fight. Once every enemy is dead, look around the room for any barrel you need to die by. Hurt yourself through any means until you are in critical condition (like 5 HP or less). Your own grenades will help a ton with this, as will other explosive weapons you have. Just make sure to take note of how much damage you take.

Once you are down to barely any health (5 or lower), stand next to the elemental barrel, and shoot it. Allow the explosion to kill you! Once you respawn, just start a new run, and play until you get to a room with another one of the barrels you need to die by. Again, don't let it get destroyed. Kill the enemies, get to low health, explode the barrel in your face! Repeat for barrel number 3, and once you have died to it, the achievement will unlock!
