To add security questions to your PS5 account, you can follow these steps:

Sign in to your PlayStation Network account on your PS5 console.
Go to Settings and select Security.
Select Security Question and Answer.
Choose a question from the list and provide an answer.
Repeat the process for the second question.
Once you have set up your security questions, you can use them to reset your password in case you forget it. To reset your password, follow these steps:

Go to Settings and select Account Management.
Select Sign In and enter your sign-in ID (email address) and password.
Select Security and then select Password.
Follow the on-screen instructions to reset your password.
