Big Boss infiltrates the research facility in order to find a cure for malaria. While moving through the facility, he sees Cunningham, mysterious girl and a man revealed to be Gene walking near one of the facility's buildings that is still under construction. As they do, girl warns both Gene and Cunningham to stop with a part of the construction scaffolding collapsing immediately afterwards, injuring one of the soldiers. Immediately, Gene has wounded soldier sent to recover and addresses everyone present in order to persuade them not to give up. Witnessing this, Big Boss realizes that Gene has unnatural charisma. As he continues to search the facility, Big Boss eventually finds himself in a room containing a tank with a young man suspended inside. Immediately afterwards, a girl named Elisa enters the room and after helping Big Boss avoid detection by the guards looking for him, reveals that she and her twin sister Ursula work for FOX Unit as a medic and psychic respectively with Ursula having been the girl whom Big Boss saw with Gene and Cunningham. She tells him that both her and Ursula were born in East Germany and were experimented on by the Soviet government due to their psychic powers before being rescued and moved to the US by Gene as part of CIA's attempt to secure children with psychic abilities from Eastern Bloc countries for their own purposes. However, while Elisa works as the unit's medic, her sister apparently became consumed by her powers, having lost her humanity in the process. Elisa also reveals that the man in the suspended animation is an experimental super soldier who was the only survivor of an experiment meant to create multiple "perfect soldiers", explaining that the tank he is held in serves to "readjust" him between assignments by wiping his memories clean through total sensory deprivation, leaving him as an emotionless husk capable only for killing. Elisa also gives Big Boss a dose of medicine needed to treat Campbell's malaria and tells him that he will find what he is looking for at the docks, ultimately helping him escape the facility.

Once back at the base, Big Boss learns that Campbell doesn't know where the base's docks are with Johnathan suggesting that they form a spy unit made up of captured soldiers and post them at the base's supply depot in order to find out the location of the docks. Big Boss agrees with the idea and sends Johnathan to the depot. Soon, he is notified that the depot contains a map showing all transport routes on the peninsula with Snake deciding to go in and retrieve it...
