Having penetrated Groznyj Grad's perimeter, Snake proceeds to track down and subdue Major Raikov in order to steal his uniform while managing to hide his face using a mask made to look like Raikov's face. Upon disguising himself and stuffing knocked out Raikov into a locker, Snake enters the facility's western wing. While investigating it, he sees disguised Eva taking a film roll from Sokolov. As she leaves, Snake enters the room, telling Sokolov that he intends to rescue him. However, with Shagohod's phase 2 testing about to begin, Sokolov has grown resigned as he is aware that Soviets will consider him a traitor for trying to defect to the US while Americans will force him to create a weapon of mass destruction similar to Shagohod, proceeding to explain Shagohod's phase 2 to Snake; It involves accelerating Shagohod with rocket boosters in order to increase its payload's maximum range enough to let it cause a nuclear strike anywhere in the US with Volgin intending to use his vast fortune in order to mass-produce them, eliminating the "balance of fear" between the US and USSR and usher in World War 3. Sokolov also reveals that the film he gave to Eva was in fact Shagohod's test data. Afterwards, Volgin enters the room with Snake managing to put his mask back on. Volgin addresses "Raikov", telling him that he expected him in his sleeping quarters before gripping his crotch, immediately realizing that the man is not Raikov. He pulls the gun on Snake but is quickly disarmed. Then, The Boss also enters the room, overpowering Snake and ripping his mask off, exposing him to Volgin who takes the initiative and sends The Boss out while he proceeds to shoot Sokolov through both kneecaps before savagely beating Snake into unconsciousness.

Snake wakes up tied in Groznyj Grad's torture chamber with a bag over his head, hearing Volgin as he tortures Sokolov for the identity of the spy operating in Groznyj Grad, ending up beating him to death. With Sokolov dead, sadistic colonel moves onto Snake, trying to learn his true motives for entering Groznyj Grad, suggesting that it may even be the Philosophers' Legacy. As Volgin tortures him, proceedings being witnessed by The Boss, Ocelot and disguised Eva/Tatyana, a transmitter falls from Snake's body. Noticing it, Volgin demands to know who planted it onto Snake with The Boss claiming to have done it to let Cobra Unit track him down. Unconvinced, Volgin demands that Boss personally blinds Snake by cutting his eyes out in order to prove her loyalty to him. The Boss agrees, however, just as she is about to strike him with her knife, Eva intervenes and stops her. Finally fed up with her, Ocelot openly outs Eva as a spy and demands permission to "test" her via his version of Russian Roulette with Volgin agreeing despite remaining unconvinced of Ocelot's claim. Knowing that Ocelot intends to kill Eva, Snake bends his restraints in order to swing between them, throwing Ocelot's aim off and causing a loaded revolver to misfire into his eye, blinding him. Satisfied with the outcome, Volgin leaves along with The Boss while Ocelot briefly stays to reveal his newfound appreciation for the torture, covertly hiding the transmitter into one of Snake's wounds. As she leaves, Eva urges Snake to meet her in the sewers below the fortress just before he is taken away. Once in the cell, Snake escapes and manages to make his way into the sewers, being contacted by Eva who tells him that Volgin has learned of his escape and sealed the sewers off in addition to sending soldiers to hunt him down, urging him to run north. As he does, Snake is backed into corner as the route ends in an outlet pipe leading into a river. With nowhere to run, Snake is confronted by Ocelot who tries to shoot him. However, his gun jams with Snake leaping into the river, nearly drowning in the process.

Snake wakes up in the middle of the lake and starts walking through it, being confronted by the spectral man known as The Sorrow who states his intention to show Snake every person he had killed during his mission, eventually letting him return.

Waking up back in the real world, Snake realizes that he has been carried downstream into the woods near Groznyj Grad. Upon re-establishing contact with his team, he learns that Sorrow was a Cobra Unit member whom The Boss killed during an assignment in 1962 and that he was a gifted spirit medium, revealing that Snake experienced a clinical death. Snake also contacts Eva who urges him to meet her at an nearby cave. Once there, Snake is given his equipment and C3 explosives meant to be used to destroy both the Shagohod and its assembly line, effectively derailing Volgin's plans...
