Make sure Geralt drinks plenty of potions with Kiss and Willow included. These potions, along with the various talents will give full resistance to stun and knockdown and high resistance to bleeding. Make sure to use enough potions to bring your toxicity up to 50 to trigger Altered Metabolism to give you further bleeding resistance and immunity to pain and poison, in addition to a bonus to attack and dodge. Together this will give you high offensive and defensive stats and resistance to all of the Koshcheys critical effects.

In combat with the Koshchey, begin by using all of your Endurance wih Igni attacks before hitting it with a full chain of Fast Silver Attacks. After finishing the chain, use up all your endurance with Igni then switch back to Fast Silver. Repeat as needed until victory. The dodge and parry bonuses from Fast style and Altered Metabolism should let you drop it faster then it can drop you.
