Block Quest V is a Breakout-style clone with a DQ-esque (Dragon Quest) title screen, not to mention 18+ and/or adult content. It was developed by Wiz in 1993. There are no real "power-ups", but there is a good variety of blocks that have varying affects on your ball and you can gain multi-balls from staying on the field long enough (the ball gets bigger and faster until it explodes, which gives you two extra balls that move slower). Wiz also felt the need to throw in a semi-useful "Screen Shaking" technique which alters the trajectory of the ball. This isn't particularly surprising as Wiz had some prior experience with pinball games and adding a sort of "tilt" feature helps when the ball seems like it just keeps flying at the same angle over and over. You can also gain hearts if you manage to get the ball to break enough tiles without returning to your paddle, which award you sizeable little bonuses.

There are nine main stages that have numerous levels within each and feature a different girl/woman and theme (Nurse/Hospital, Student/School, etc.). As you beat various levels within a stage, you'll be rewarded with pictures, which get progressively more mature in nature. It's a simple game handled pretty effectively by Wiz, though there isn't a whole lot to say other than that. The game has good music and good presentation considering what this game specializes in.

This is a video skipping the first stage (a little too simple) to show the first few levels of the second stage, ending with the first picture you get on the stage. To my knowledge, there aren't four BQ games prior/related to this. Enjoy.
