The final level in this game, and this walkthrough.
It's a lot shorter than TNT: Evilution's equivalent, but it doesn't make it any easier. The main factor is the Cyberdemon. You need to kill it quickly so that too many enemies don't spawn in. It's position kind of makes it hard to kill it quickly, in a safe manner. Launch a few BFGs from the other side of the pit and then finish it off up close and grab the Megasphere. Once I fgured out the timing for hitting the Icon Of Sin, it wasn't really hard to finish.
It's definitely been the hardest 'real' DOOM game I've played, and probably the most fun too.
