To use the retinal scanner in Metal Gear Solid 2, you first need to subdue a guard. To do this, stand still behind a guard without a weapon equipped, then choke. While in a hold, move to the door and spin so the front of the guard faces the scanner.

The guard has to be awake for this to work. If you have knocked out every guard in the area, use the lift and return to reset the sentries.
