You can drop through the walls of the Town Hall and reach the underground lair early to go fetch the Ceremonial Dagger. Unfortunately you can't do this at the start of the game, because the lair's innards are still unloaded. Instead, you have to wait until you've returned from Sewers to be able to do this.

So basically, you skip ever seeing Doctor Fitch and the entire Hell Descent, plus a little running around. A decent improvement over the "Skip (most of) Hell Descent" skip.

There's a clip at the end of how it looks like if you attempt to drop to the underground lair early in the game... The collisions are there, but it doesn't look like you can examine anything. In this clip I drop to another spot, though, running a bit on the invisible ceilings of the lair's corridor.
