Most AAA videogames have a lot more bugs filed on them then you may think.
Where do they all come from? And How do you manage that many?

Bugs come from a variety of places: QA/QV, Automated testing, and the devs themselves.

You want to encourage the filing of bugs
But you also want to have ways of dealing with bugs that (for a variety of reasons) aren't going to get fixed.
Whether they be duplicates, not really bugs, or otherwise.

Game development isn't just about making things, its also about finding (and fixing) the issues).
