I've always liked the mad challenge that was to complete the first 2 levels of episode 4. This first map is challenging because it's a pistol start and there's virtually no health. The second map is challenging because you might start with very low health, coming from the first map. Either way, fast monsters mode takes the challenge to a whole other level.

Thanks NightTerror for the route idea of going head-first to the secret as soon as the map starts. It's a slightly risky strategy because of the shotgunners, but it saves some time, and if done properly the risk can be minimized, making each run attempt more consistent and increasing the rate of survival. The "proper" way to do it is to use the shotgun pickup to switch to the fists, and then use the rocket launcher pickup to switch to the shotgun, so that when you come out of the teleporter you can instantly take a shot and whatever is in front of you.

The upper platform that you teleport to is quite a good vantage point where you can kill the remaining hitscanners from cover, then you can press the button to move forward and use the specters as meat-shields from the shotgunners on top of the lift, which also helps killing them faster. The red-key grab trick saves time, since you can immediately go back to the red door.

The biggest problem with the red door room is collecting the 2nd secret properly without triggering the trap-walls. To do that, you need to step on the lowering platform before it shows the "NIN" letters, to guarantee that you tag the secret. Then strafe to the left side before going forward, because the "front" side of the "NIN" letters is what triggers the walls to rise.

After that, getting a good time is a matter of being lucky with the monster movement, to get all 5 Barons grouped together when you start using the rocket launcher. That part did not go very well in this run, and I believe that this strategy, if perfected and with a bit more luck, can yield a time below 2m10.
