Pilot error and inadequate training were factors causing the mid-air collision of two Red Arrow jets last year, crash investigators have found. Flight Lieutenant Mike Ling, 31, was injured in the incident which also involved his colleague Flight Lieutenant Dave Montenegro during training in Crete last March. The move the pilots carried out, where the planes were attempting to cross flight paths, has been banned until the risks into it are further probed. The report found the crash was caused by Flt Lt Montenegro's decision not to pull out of the manoeuvre when he became aware of a problem. "Both pilots reacted instinctively rather than conditionally to this unfamiliar situation," crash investigators stated.
