In total there are 13 Heart Containers to get in the game, however, 8 of them are gotten from defeating the bosses in dungeons 1-8. The remaining 5 are hidden across the overworld. This video shows how to get these 5 during the First Quest.
In order to get them all you'll need to have the Raft from Level 3, the Stepladder from Level 4, bombs, & 1 of the Candles. For help on how to get these items then watch their corresponding videos linked down below.

Heart Containers:
#1 (0:00): From the starting screen go right 4 screens until you see water. On the north wall there is a flat part to the left of the road leading up. Drop a bomb near the second square from the right of this flat part to open a cave. In the cave you'll find an Old Man offering you the choice between a Life Potion and a Heart Container. DO NOT choose the potion. If you do you will permanently lose access to this Heart Container (this is the case for all of the Old Men that will give you this choice). Take the Heart Container like a good hero & continue on your way.

#2 (0:40): From Heart Container #1 go right 4 screens & up 2 screens. This Heart Container will be readily apparent sitting on a dock in the middle of the ocean. Use the Stepladder to walk over to it and grab it.
Directions from the starting screen are: right 8 screens & up 2 screens.

#3 (1:13): From HC #2, go up 2 screens & you'll see a dock. Step on the dock & use the raft to go upwards. Enter the scary statue's mouth & you'll find another Old Man offering you a Heart Container.
Directions from the starting screen are: right 8 screens, up 4 screens, then use the raft to go up once more.

#4 (1:36): From HC #3 go down 1 screen, left 1, up 1, & left 2 more. There will be a big rock in the middle of the screen. Use a bomb to blow open a cave at the bottom of this rock at the rightmost square to find yet another Old Man.
Directions from the starting screen are: right 1 screen, up 5 screens, & right 4 more screens.

#5 (2:11): From HC #4 go left 4 screens, down 2, & left 1 screen. Here you'll find a ton of bushes. Burn the bush 5th from the right with a candle. This reveals a cave with another Old Man. If you have trouble burning the bush then step back a bit & try again.
Directions from the starting screen are: right 1 screen, up 3 screens, & left 1 screen.
