In order to unlock the Inflammatory Actions achievement, you must set a Headcrab on fire (not a Headcrab Zombie, an actual Headcrab). I found a place to do this in the third chapter (but I am sure there are other places throughout the game this can be pulled off).

Start Chapter 3, Unforeseen Consequences, and play through the level until you have to climb up the ladder where the elevator falls. At the top, you will meet a guard, and he will kill the enemies nearby if you have him follow you. Once the enemies in the room are dead, go to the other side of the room where a dead body can be seen with a few flares next to it.

Go through the door past the corpse, and continue following the path until there is a short hallway to the right (if you go past it, you will find the breakroom, locker room and bathroom). Go down the hallway to the right, and then turn left and follow the path. A Headcrab Zombie will throw an object at you. Go past this enemy, and you will be in the Lobby from the beginning of Chapter 2.

Once in the Lobby, grab the flare from the desk and then quickly duck under the half-open door. There will be some broken windows here (where the guard opened the door for you in the second chapter), and you will be attacked by a Headcrab. Simply walk into the Headcrab to light it on fire, and once it dies from the fire, the achievement will unlock!
