In order to unlock the Got Out Of Hand Quickly achievement, you must find, and kill, the Crawling Hand. The Crawling Hand can be found on the ground level. It will crawl out of the bus that crashes into the building. To access the ground level, you must use the Earth and Mars discs on the circular objects in the stairwell.

Once the bus has crashed into the building, the Crawling Hand will be seen climbing out of the front of the bus. Once it has crawled out and is walking around, walk into it to initiate combat. Kill it, and the achievement will unlock!
