In order to unlock the Burned Out achievement, you have to find, and kill, the Furnace. The Furnace is found in the middle of the Sewer area of the Basement, but in order to fight it, you must first kill all of the Tick enemies in the area that are feeding off of the pipes.

I lost my video where I showed where all of the Ticks are, but it is not hard to find them. Just look for any pipes still glowing blue, and head in the direction of the to find the Ticks. Once you kill a tick, the glow on the pipes changes to red/orange. From where the Furnace is, you can see where all the pipes lead, so this is the best spot to see if you have any blue glowing pipes remaining.

Once all of the Ticks have been killed, and the pipes are no longer glowing blue, head to the center of the Sewer. From the stairwell entrance, go east through the first door, south, east twice, and then south. You then need to jump down to the lower floor. Once you jump down, head west a few screens and then jump down when you have to. Go west one more screen, and this is where the Furnace is. Once you have killed all of the Ticks, walk into the furnace to initiate combat, kill it, and the achievement will unlock! You will also get the Furnace Edge melee weapon, which is pretty strong!
