Here's how to forget a PS4 controller from your console's Bluetooth settings and then reconnect it:

Part 1: Forgetting the Controller from PS4 Bluetooth Devices

You'll need another way to navigate your PS4 for this, as the controller you're trying to forget won't be responding to the console.

Option A: Use a different, working PS4 controller. (Easiest if you have one.)

Option B: Use the PS4 Second Screen App. Download "PS4 Second Screen" on your smartphone/tablet and connect it to your PS4 console via Wi-Fi. This app allows you to navigate the PS4 menus.

Option C: Use a USB Keyboard/Mouse. You can plug a USB keyboard and/or mouse into your PS4 to navigate the menus.

Steps to Forget the Controller:
On your PS4, go to the Home Screen.
Navigate to "Settings" (the toolbox icon on the top row).
