In order to unlock the Pixel Perfect Achievement and Artifact, you must go to settings, and change a few options. The Artifact grants the Player a x0 bonus to score, and when equipped, makes the game very pixelated during your run. The following options are the 2 that need to be changed in Settings:

1) Set Resolution to the lowest possible resolution you can.
2) Set Window Mode to Windowed.

Once these two conditions are met, start a game. Now, all you need to do is allow yourself to be killed, and you will unlock the Pixel Perfect Achievement and Artifact!

Update: some people are reporting having to adjust the render scale slider down to 0% so if it doesn't work, try that as well
