Compared to the Master System version, the Game Gear version seems to have been finished a bit later. It has a little extra polish to it, like better drawn menus and some different color palettes. Screen crunch doesn't hit this game as bad as it did to Aspect's Sonic 2, but some bosses are a bit harder. Luckily they wised up and let you collect rings in their levels.

Speaking of bosses, the Dangerous Ball Tower fought in Gigalopolis Zone shows up in Sonic Mania, in Mirage Saloon. I can't say I was expecting that to ever happen!
